Primary Care and Psychiatry Shortage Grant
The Primary Care and Psychiatry Shortage Grant, (Act 128), was created to encourage primary care physicians and psychiatrists to practice in underserved areas of the state of Wisconsin.
Physicians and psychiatrists, who are interested in receiving financial assistance under this program, must complete and submit a Notice of Intent to Practice in an Underserved Area in the State of Wisconsin form prior to accepting employment or becoming in any other way affiliated as a physician in an underserved area in the state of Wisconsin.
In the spring of 2015, individuals who submitted a Notice of Intent to Practice form will be invited, via email, to complete and submit a Claim for Assistance. The information collected on the Claim for Assistance will allow HEAB to determine who is eligible for financial assistance. Eligible recipients will be notified of the financial assistance award amount by the summer of 2015.
One of the eligibility criteria for this program is that the physician practices in an underserved area of the state of Wisconsin. For a list of underserved areas, please visit the U.S. Department of Health and Human Service's website at: http://hpsafind.hrsa.gov/.
